AIM Reports

Purpose

An AIM report is a formal recording of the facts related to an accident, incident, malfunction and/or equipment irregularities. The report usually relates to an accident (any occurrence resulting in injury requiring medical attention or a fatality), or incident (any occurrence which could have resulted in a situation leading to injury or fatality) that has occurred. It also pertains to any unusual occurrence where a partial or complete malfunction of the equipment may have led to the initiation of emergency procedures. An Equipment occurrence shall be considered as any irregularity related to equipment that occurs outside of the physical phases of a skydive that could have resulted in an Accident, Incident, or Malfunction. The sole purpose of the AIM report is to enhance safety and assess preventative measures.

Any incident that involves skydiver, staff, and/or customer safety should be recorded, no matter how insignificant it may seem.

We strongly recommend the DZ retain a copy of any DZ-submitted AIMs for their records.

CSPA requires immediate notification in the event of:

  • a major accident/injury
  • fatality; and/or
  • 3rd party damage or loss

And in any case, an AIM report must be filed within ten (10) working days of any occurrence.

Choosing the Most Accurate AIM Report Form:

  • Tandem AIM Form: Documents occurrences involving a tandem instructor and/or tandem student, including, but not limited to, landing injuries, deployment malfunctions, or external factors affecting safety.
  • Student AIM Form: Documents occurrences involving student jumpers and/or instructors, including, but not limited to, incidents during freefall that compromised the safety of the skydive, landing injuries or accidents, or deployment malfunctions.
  • Experienced AIM Form: Documents occurrences involving licensed/certified skydivers, including, but not limited to, landing injuries or accidents, deployment malfunctions, or incidents during any phase of the skydive that directly affected their safety and/or the safety of others.
  • Equipment Irregularities AIM Form: Documents occurrences related to equipment irregularities identified outside the physical phases of a skydive that could lead to an accident, incident, or malfunction. Examples include, but are not limited to, excessive line wear, faulty components, or packing errors like a misrouted bridal.

General Guidelines for Completing an AIM Report:

Each AIM report should be completed as thoroughly as possible, ensuring that all relevant details are accurately documented. A well-detailed report helps in identifying trends, assessing risks, and implementing effective safety measures. Providing comprehensive information -including incident specifics, contributing factors, witness accounts, and equipment details -allows for a clearer understanding of the occurrence and supports informed decision-making to prevent future issues. Additional recommendations include:

  • Ensure Accuracy: Provide clear, factual, and concise details about the event.
  • Use Objective Language: Avoid speculation where possible
  • Include Witness Statements: If applicable, gather statements from those involved or who observed the occurrence.
  • Attach photos or other documentation supporting any equipment issue.
  • Submit the completed AIM report promptly to aim@cspa.ca
  • Maintain a copy of submitted AIM report(s) for your records
